Please come to your appointment 15 minutes early to fill out required paperwork or print out the PDF forms below and have them filled out. Our physicians make every attempt to see you at your scheduled appointment time. Parking at NWH at times may take 15 minutes or more. In consideration to all our patients, those arriving 15 minutes after their scheduled appointment time may be rescheduled.

Health Insurance Information
We understand that medical insurance can sometimes be intimidating and confusing to the patient. Our staff will do their best to help you understand the insurance process, and will be glad to answer any questions that you might have. Some companies pay fixed allowances for certain procedures, and others pay a percentage of the charge.
It is your responsibility to pay any deductible amount, co-insurance, or any other balance not paid by your insurance company.
If your insurance carrier requires that you obtain a referral from your primary care physician, it is your responsibility to provide us with a valid referral number at the time of your visit. If no referral number is available, you will be asked to pay for any services rendered at that visit.
